I hereby affirm that I am voluntarily starting a course of instruction in physical fitness and performance training (The Activity). I am voluntarily participating in the Activity entirely at my own risk.
In full consideration of the risk of injury while participating in the Activity, and for the right to participate in the Activity, I hereby, for myself, my heirs, executors, administrators, assigns, or personal representatives, knowingly and voluntarily participate in this waiver and release of liability and hereby waive any and all rights, claims or causes of action of any kind whatsoever arising out of my participation in the Activity, their affiliates, managers, members, agents, attorneys, staff, volunteers, heirs, representatives, predecessors, successors and assigns, for any kind of risks related to traveling to and from as well as participating the Activity, which may include, but are not limited to, physical or phycological injury, pain, suffering, illness disfigurement, temporary or permanent disability, economic or emotional loss, and death.
I acknowledge that I have carefully read this form and fully understand that it is a release of liability. I expressly agree to release and discharge the trainer or instructor from any and all claims or causes of action and I agree to voluntarily give up or waive any right that I may otherwise have to bring a legal action for personal injury or property damage.

Welcome to your Personal Training Fitness Program. But before we begin, we would like to inform you about the purpose, risks, and benefits of the program. This will help you understand more about the personal training as we run through these details including your rights and responsibilities in order for you to reap the full benefits of the program.
PURPOSE
This personal fitness training program is recommended for the improvement of one's physical fitness through a form of physical and cardiovascular exercises. This includes the improvement of dietary intake, stress management, and overall health management.
You will be given exercise programs and instructions that are suited for your physical body and you will have a physical fitness trainer to monitor your progress.
After every session, you will be assessed after you provide your evaluation of your activity in order to monitor your progress.
In case you are taking any medications, please let us know right away.
RISKS
There are chances that during the period of work-out, adverse changes might occur such as dizziness or fainting. These might be due to an abnormal increase in blood pressure. In rare instances, it may lead to a heart attack or stroke which may be a cause of death. Sometimes, injuries during participation of the exercise programs might happen that may involve the joints, ligaments, muscles, and tendons. It shall be your responsibility to ensure the prevention of these during the workout. You do not need to push hard if your body cannot make it through yet. Take time and let the progress come in time, gradually.
BENEFITS
Joining the program helps you know the correct posture for your exercises and will help quantify your progress. You can learn and understand more about the science of your physical health as well and how to utilize these effectively
Program benefit is dependent on your adherence to the defined program.
CONFIDENTIALITY
Your privacy is important to us. All information we obtain from you before, during, and after the fitness training program shall be kept confidential and will not be released to anyone without your prior written consent. However, we may use data that does not personally identify you for statistical purposes.

By agreeing to this digital document, I hereby declare that I have read the information above and voluntarily participate in this personal training fitness program.
I understand the risks and benefits of these physical activities as well as my right to privacy. I also have had the opportunity to ask relative questions concerning the program and all of which were explained to me and to my satisfaction.

1:1 TRAINING FREQUENCY + DURATION
Minimum training session commitment for 1:1 clients is twice per week for 6 months unless otherwise agreed upon. In the case of personal, familial or medical emergencies this commitment may be modified.
CANCELLATIONS + RESCHEDULING
Please provide 24h notice for 1:1 and group cancellations. Late cancellations will result in being charged for the missed session. 1:1 sessions cancelled with 24h notice can be rescheduled to take place within 7 days of the original session date.
If a 1:1 client cancels or otherwise misses 50% or more of their sessions in a given month, a consultation session is required to update their training plan.
LATE PAYMENTS
Client invoices automatically incur a 10% late fee if not paid within 7 days after their due date.
If a client’s invoice remains unpaid for 30 days, Well and Strong LLC has the right to terminate their agreement with the client.
SPECIALIST REFERRALS + SCOPE OF PRACTICE
Well and Strong LLC helps clients achieve fitness adaptions. We are not qualified to diagnose or treat chronic pain or injury.
If a trainer determines that a client is in need of specialized care beyond Well and Strong LLC’s scope of practice, Well and Strong LLC has the right to terminate their agreement with the client and refer them to the appropriate specialist.
